Quick Note #19: Redesigning Retigabine to Avoid Previous Side Effects

A Kv7 channel-targeting drug called retigabine was pulled from the market years ago after it resulted in some skin discoloration. Since then, it has been reformulated as azetukalner to overcome this side effect, and it is now showing promising results in adults with focal epilepsy.

What It’s About
Retigabine helps Kv7 channels stay open longer, which can calm down overactive neurons. Because of this, it was prescribed as an antiseizure medicine, but after a couple of years on the market, some patients developed a bluish discoloration, and its use was discontinued.

By looking at the chemical structure of retigabine, researchers were able to determine that it wasn’t the drug’s action on the potassium channel itself that was causing the issue. Instead, a specific part of its structure broke down into a byproduct that caused the side effect.

They used that insight to build a new version of the molecule, azetukalner, that keeps the same channel-opening effect but removes the problematic piece. Azetukalner has moved through Phase 1 and 2 clinical trials and has recently completed Phase 3 testing in adults with focal epilepsy. Early results are encouraging, showing seizure reduction without the blue discoloration that limited retigabine’s use.

Why It Matters
Although azetukalner isn’t being developed for KCNQ2-related disorders specifically, it targets the same protein many KCNQ2 variants affect. Showing this channel can be safely targeted by a drug is a key step toward future treatments developed specifically for KCNQ2-related disorders.
